A referral is a request for UKeff to assess the client’s circumstances. It does not guarantee eligibility, support or any particular form of assistance.
Do not promise or advise a client that UKeff will provide a supermarket voucher. UKeff will decide whether assistance is appropriate and, if so, what form it will take. This may include a crisis food parcel, essential items, advice, signposting or, in exceptional circumstances, a supermarket voucher.

Emergency food referrals require complete financial evidence. This includes original PDF statements covering at least the most recent 30 consecutive days for every relevant account, together with the latest full Universal Credit statement where applicable. Photographs, screenshots, cropped pages and transaction lists will not be accepted as bank or Universal Credit statements.
This referral form is not an emergency service. Follow your organisation’s safeguarding procedure and contact emergency or statutory services where required.
Complete every section displayed below. Required evidence must be supplied as an original downloadable file. Where a fact has not been verified, say so clearly. A referral does not guarantee assistance.
For an emergency food assessment, supply original PDF statements covering at least the most recent 30 consecutive days for every relevant account. For another financial request, including a grant, essential household item, carpet or flooring request, supply the most recent three complete months up to the date of referral. If more than one financial pathway is selected, the three-month requirement applies.
Statements must be complete downloads and show the account holder’s name and account details. They should show the current address where the provider includes it. If the provider’s PDF does not display the address, supply separate proof of the current address or explain why this is unavailable. For a client with no fixed address, record the position rather than entering a false address.
Also supply the latest full Universal Credit statement where applicable, evidence of other income and the equivalent information for a partner or other adult who shares finances or essential household costs with the client. Screenshots, photographs, cropped pages, edited documents and transaction-only lists are not accepted in place of complete statements.
UKeff may request clarification or further evidence. Documents are collected for assessment; they do not create an entitlement to support.
Emergency food referrals are assessed individually. UKeff does not automatically issue supermarket vouchers. Depending on eligibility, evidence, availability and practical need, UKeff may offer a crisis food parcel, essential items, advice, signposting or another form of assistance.
Declining a suitable food parcel does not create an entitlement to a voucher. Explain any genuine reason that a parcel would be unsafe or unsuitable.
Funding for carpets, flooring and household items is extremely limited. Eligibility does not guarantee that an item is available. UKeff may require a client contribution and may place eligible clients on a waiting list.
The referrer should first explore the landlord’s obligations, Local Welfare Provision, reuse schemes, supplier support and other appropriate sources.
UKeff does not automatically provide clothing vouchers. Where available and appropriate, UKeff may offer suitable new or good-condition clothing items. Give accurate sizes and select only items currently needed.
Important: The privacy wording in this working copy must be approved by UKeff’s data-protection lead before publication, including lawful bases, any special-category condition, retention, sharing and the linked privacy notice.
Do not upload or submit another adult’s financial documents without an appropriate lawful basis. Remove those files and contact UKeff for guidance before proceeding.
UKeff will use the information in this referral to assess the client’s needs and eligibility, communicate about the referral, deliver or coordinate support, prevent duplicate or inappropriate awards, safeguard people where necessary, keep appropriate records and meet legal or funding obligations.
Information may be shared with the client, the referring organisation and relevant services where this is necessary, lawful and proportionate. UKeff will not use this form to obtain blanket permission for unrelated future contact.
Financial, health, domestic-abuse, immigration and other sensitive information should only be included where relevant to this referral. For full details about who controls the information, the lawful bases used, retention, individual rights and how to complain, read UKeff’s current Privacy Policy.
Do not include unnecessary information about another person. Do not upload passwords, PINs, full payment-card details, access codes or online-banking credentials. Redact information that is not relevant, but do not crop, alter or remove transactions from required financial statements.
